The Vivify Pathways TM solution is designed to improve the efficiency and effectiveness of disease management and post-acute care programs. We do this by leveraging a cloud-based virtual platform utilized by providers and payers. Vivify Pathways collects data from patients through their mobile digital devices or at-home remote monitoring kits. The acquired biometric and patient supplied information provides actionable insights for clinicians for more timely interventions

How it works

Providers choose whether to onboard patients at their healthcare facility or ship Bluetooth-enabled, third-party remote patient monitoring devices directly to patients’ homes. These devices, which may include a blood pressure monitor, pulse oximeter, weight scale, glucose meter, pedometer, and thermometer (additional devices available and additional charges may apply), are assigned based on the clinical condition(s) pathway of each patient. Vivify Health provides help with setup.

Accessing +Go

Patients use their mobile phones to access +Go by downloading the Vivify Health app from the app store (available for iOS and Android devices) and following instructions provided by their healthcare providers.

+Go Guide supports patients or members who need more guidance with their health or condition by providing information on selected chronic conditions, enabling patients/members to enter prompted biometric readings, tracking results, and providing tailored feedback based on their entries. The patient can securely message their healthcare team via the app and the team can remotely monitor responses and biometric entries and respond securely to patient’s in app message requests.

+Go Engage

+Go Engage enables convenient connections between patients and providers outside of the office. The app keeps them engaged with your health system and provides daily (or as-needed) health-related information and follow-up reminders to make appointments and fill prescriptions. You can also use +Go Engage to reinforce discharge instructions through health tips, articles, and videos.

+Go Monitor

Patients at a higher risk for complications or who need involvement with their healthcare team turn to +Go Monitor. This level of service provides daily (or as scheduled) health education via our Smart Condition Specific Pathways and enables the healthcare team to securely message or video visit with the patient. Bluetooth-enabled devices are available to self-pair with the patient/member’s phone for ease of biometric entry, trending of biometric results, and alerts in the Care Team Portal to highlight results for review by the healthcare provider.
